How it works:
To stay in compliance with certain manufacturers distribution policies, and to provide added safety for your pet, RxPetfood.com requires veterinary authorization for this product.
Please print out this form and fill out the top portion. Give it to your veterinarian to fill out the bottom and fax to (866) 447-3879.
If you have already obtained a written authorization, attach it to the form and fax that to us.
If preferred, we will contact your vet for you. Simply choose "No, Please call my vet for authorization" on the drop down menu when selecting your product.

Common Questions:
Q: Should I fax authorization before or after I place my order?
A: Either. We prefer that the order be placed beforehand, however, it isn't required. Authorization calls or faxes from you or your vet will be applied to your order regardless.
Q: Do I need to obtain authorization from my vet every time I order?
A: No, most vets authorize multiple or infinite refills. Refills are recorded and applied to future orders.
Q: The website requires my veterinarians' information every time I place an order. Do I really need to fill the form?
A: No, if you have refills on file from a previous order, you can type " REFILL " in the information feilds and select the option, "No, I Have Refills On File", when adding the product to the cart.
Q: Do all foods require veterinarian authorization?
A: No, "Hills Science Diet" and "Waltham Royal Canin Diet" brands require veterinary authorization.
Q: How long will it take to authorize my order?
A: Most orders are authorized the same day they are received. Refill orders are authorized automatically. You can call your vet beforehand to ensure quick authorization. >
